CONSIDERATIONS – Class 8
1. Small group interaction then role play. Three groups and each group assigned one of the following roles: Hospital wanting to form a “PHO”,Physician Group wanting to enter into a contract with a Management Service Organization, and a New Doctor evaluating opportunities in joining a PHO and in joining an IPA which is looking at entering into an agreement with a Management Service Organization. Each group will appoint a recorder to take notes of the discussion from the assigned perspective. Then join back together as one group and only one person from each individual group presents assigned role.
ROLE PLAYING:
Group “A” is a hospital
Group “B” is an Independent Practice Association of physicians
Group “C” is a new doctor
(1) Group “A” is a hospital that has a small Physician Hospital Organization (“PHO”) and is trying to convince Group “B” to join its “PHO”. List reasons why Group “B” should become part of the “PHO”.
(2) Group “B” is replying back to the hospital that it wants to retain its Independent Practice Association (“IPA”) status and that it wants to work with the hospital through the services of a management service organization. List reasons why Group “B” wants to maintain its independent status and at the same time convince the hospital of why the relationship will work.
(3) Group “C” is a new doctor who has been approached by both groups, Group “A” to join its “PHO” and group “B” to join its “IPA”. List what the new doctor should consider in evaluating opportunities with both groups.
2. Practice Acquisition – Dissolution. Consider the following scenario: You are part of a medical group practice that has been acquired by a hospital. About five years after being an employee of the hospital pursuant to the acquisition of your medical practice by the hospital, the hospital has notified you and all other members of your medical group that due to continuing financial losses involved in owning medical group practices, the hospital no longer wants to be in the business of owning medical group practices. The hospital has notified you and all other members of your medical group that it will not be renewing its current employment contract with each member of the group and that the hospital will instead immediately pay each member of the medical group the remaining six months in full of the employment contract in order to immediately terminate the employment relationship amicably. (1) What do you do? What questions do you have? (2) Consider that the medical group decides to become an independent entity yet no one from the medical group volunteers to be the physician administrator for the group, what do you do then? (3) Consider that you end up volunteering to be the physician administrator for the group, even though you are a leader in the group with having excessive amounts of patients wanting your services. What do you do? What questions do you have? (4) Consider how you handle the relationships with the members of the group that feel like they should be taken care of and are entitled to continue with their 30 hour work week as they had under their employment contract with the hospital and they do not want any administrative risks or duties.
